Attention supervisors and employees:
Please note that a Separation Form must be completed for any employee (staff or contingent) at least two weeks before the last day the employee is on payroll for transfers, resignations, retirements or termination from employment. The Separation Form must be submitted to the Office of Human Resources (OHR) two weeks prior to the employee?s last day of work. Additionally, if the employee had supervisory responsibility and/or approved timesheets, OHR must be contacted with the name of the new supervisor/approver for this assignment.
