The Materiel Management Service Request form (MMSR) has been revised. The most notable additions concern a departmental budget code and computer disposal requests.
Though most of our services are provided at no cost, the departmental budget code has been added to the form primarily for tracking and retrieving purposes. Only services such as particularly large moves or non-routine requests including off-campus jobs will be charged, and departments will be informed of any costs associated with requests in advance.
Because of the frequency of computer disposal requests, we have also added a new check-off box to indicate that computers have been properly prepped for disposal per university policy. No longer will you have to type this out in the Special Instructions area of the form. This check-off certification should not be taken lightly though, as it protects the university and individuals from potential legal liability from software copyright infringement and the disclosure of personal information. The revised form is available in the forms repository. Please download a copy of the new form to your own documents file for easy access and delete all copies of the old blank form.
