Employees and spouses enrolled in the state of Maryland medical benefit plans can save money in 2017 by participating in the wellness program.
Participation is voluntary and there are rewards for completing the wellness activities.
To reap the maximum benefit, complete the activities now by following the easy steps below:
3 Steps to Qualify for the PCP Copay Waiver:
Step 1: Visit your medical carrier’s website to create an account (if you have already created an account, you do not need to do this again to complete the activities.) Once the account is created, click on the Wellness Program box.
Step 2: Once in the wellness section, select your PCP (if already populated, skip this step).
Step 3: Complete the 2017 online health risk assessment for either CareFirst or United Health Care, or the 2017 Total Health Assessment for Kaiser Permanente.
That’s it! There is no Provider Notification Form and no further action needs to be taken. When you complete the online health risk assessment, your medical carrier will be place you in the PCP Copay Waiver status category within 30 days of completing all 3 steps.
2 Steps to Qualify for the Specialist Copay Reduction to $25:
Step 1: Take the list of health screenings to your physician.
Step 2: If your doctor recommends any of the screenings, complete the screening(s). Within 30 days of completing the screening(s), your Specialist copays are reduced to $25. This happens automatically, and there is no additional action to take!
