Please note that an updated Towson University Separation Form is available in the Forms Repository. Please dispose of any old forms that might have been saved, or copied. The revised form must be downloaded for any regular Exempt; Non-exempt; Contingent I & II employee (faculty not included) at least two weeks before the last day the employee works prior to a resignation, transfer, retirement, or separation from university employment. The Towson University Separation Form must be completed, and submitted directly to the Employee/Employer Relations Unit in the Office of Human Resources (OHR) no later than two weeks prior to the employee’s last day of work. Additionally, the resignation and/or transfer letter must be sent with the Separation Form to the OHR within the above timeframe. Moreover, the Exit Survey can now be completed by the employee separating from the university on-line.
Supervisors: if the employee separating had supervisory responsibility and/or approved timesheets, the OHR must be contacted with the name of the new supervisor/approver for this assignment.
