As communicated during Open Enrollment, employees enrolled in TU health benefits are asked to participate in the new state of Maryland Wellness Program. Each year employees will be asked to complete healthy activities and will receive a reward for compliance. More information is listed below.
2015 Healthy Activities
Faculty and staff and covered spouses enrolled in CareFirst BlueCross BlueShield, Kaiser Permanente, or United Healthcare plans must complete the following healthy activities by September 30, 2015 to avoid a premium surcharge in 2016:
1. Designate an in-network primary care physician (PCP) for yourself & spouse (if applicable)
A PCP can be a general practitioner, a doctor who practices family medicine or internal medicine, an OB/GYN, GYN, physician’s assistant, or nurse practitioner. If you do not have a PCP, you may search your medical plan’s dedicated website for an in-network physician.

2. Complete a health risk assessment – available on your medical plan website or by calling your medical plan
A health risk assessment is a questionnaire that asks about your age, your diet, how much you exercise, and whether you use tobacco or alcohol. Please use the Wellness Program Activity Cheat Sheet to help you create an account on your medical plan’s website which you will need if you access the health risk assessment via the plan website.
3. Meet with your PCP to discuss the health risk assessment results, complete the physician verification form and submit it to your medical plan
Be sure to take the results of your health risk assessment and your physician verification form to your doctor’s appointment. Please follow your plan’s instructions regarding physician verification.
2015 Healthy Reward
Once you complete the healthy activities, your PCP visits for the remainder of 2015 will be covered WITHOUT a copayment. The deadline to complete the 2015 healthy activities is September 30, 2015, but the sooner you complete the activities, the sooner your reward is processed.
After you complete the activities and submit the physician verification form, your healthy reward will be processed in accordance with the Wellness Program Zero Copay Process for each medical plan.
