The OHR receives frequent calls and emails requesting benefits and payroll forms to be sent via interoffice mail. Most of the forms can be found by signing into My TU and clicking on “Benefits Forms” on the right of the portal menu. This will connect you directly to the OHR Benefits Information. You can also access the forms directly at http://www.towson.edu/adminfinance/hr/benefits/summaries.asp.
All forms are grouped by subject, including updating beneficiaries, enrolling or making changes to your health benefits, reimbursement forms for flexible spending accounts, and other general benefits forms.
These forms were placed on line to provide employees with a more efficient way to gain access to them. Please begin to use this method when requesting a form; if you are not able to locate a form that you require, we will be happy to assist you at that time.
