2004 Flexible Spending Accounts (FSA) for Health Care and/or Dependent Care expenses close on Friday, Dec. 31.
Regular faculty and staff members who have FSAs now will have until April 15, 2005 to submit their requests for reimbursements from their 2004 account(s) but ONLY FOR ELIGIBLE EXPENSES INCURRED DURING 2004! Remember that any 2004 FSA balances not requested for reimbursement by the April 15, 2005 deadline will be forfeited to the state.
Statements showing account balances and activity as of the end of the third quarter of 2004 have been distributed to all participants directly from CompuSys/Erisa Group, Inc., the state?s FSA administrator. Forms for reimbursement are available from the TU OHR, can be downloaded from Erisa?s Web site at {http://www.flexmd.org} or can be copied from the originals on pp.56-57 in your 2004 State of Maryland Summary of Health Benefits booklet.
