Attention supervisors and employees:
Please note that a separation form must be completed and submitted to the Office of Human Resources at least 2 weeks before the last day an employee is on payroll in the event of transfers, resignations, retirements or termination from employment. Additionally, if the employee had supervisory responsibility and/or approved timesheets, the Office of Human Resources must be contacted with the name of the new supervisor/approver for this assignment.
