The new Leave Correction Form has been created for regular and contingent faculty and staff who use the online time sheet system. Use this form to request a modification to either leave used or leave balances. Please follow the instructions below:
- Complete the form; sign and date (eSignature is acceptable).
- Have your supervisor review and approve (eSignature is acceptable).
- Submit the form to leavebenefits@towson.edu.
- Corrections to the type of leave recorded on your time sheet should be requested within two pay periods of the error.
This form must be used for all revisions; the Employee Benefits unit will no longer accept emails to request corrections.
